The Board received a report of the Chief Operating Officer, NHS Trafford Clinical Commissioning Group providing an update of the progress of the Better Care Fund for Trafford and the progress of the schemes.

 

The report highlighted the work of the Better Care Steering Group. It was confirmed that the Joint Finance Review was near completion and would be presented at the next meeting of the Health & Wellbeing Board.

 

Members discussed the Community Nursing scheme and how work was ongoing with Pennine Care to refocus this. Members also discussed the Intermediate Care scheme and the plans in place to increase rehabilitation bed capacity.

 

            RESOLVED: That the report be noted.

The Board received a report of the Chief Operating Officer, NHS Trafford Clinical Commissioning Group providing an update on the progress of the Better Care Fund for Trafford and the progress of the schemes. The report included a performance update on the Better Care Fund Key Performance Indicators, and confirmed that the steering group continues to meet on a monthly basis.

Minutes:

The Board received a report of the Chief Operating Officer, NHS Trafford Clinical Commissioning Group, providing an update on the progress of the Better Care Fund for Trafford and the progress of the individual schemes.

 

It was confirmed following approval by NHS England, that Trafford CCG and Trafford Council are now making progress to deliver the schemes. It was also noted that the joint financial plan is currently being finalised, and it was hoped that the report would be ready to be presented to the Health and Wellbeing Board at the next meeting.

 

            RESOLVED: That the report be noted.

The Board received a report of the Associate Director of Commissioning, Clinical Commissioning Group providing an update of the progress of Trafford Better Care Fund and the next phase.

 

It was confirmed that the programme has now received full approval and progress has been made on all the individual schemes. Up to now the Clinical Commissioning Group had only been able to provide the Board with information relating to the process, but going forward the aim will be to provide more detailed information based on ongoing reports for each individual scheme. The Chairman advised that updates on these could be staggered over future meetings of the Board.

Meeting: 06/01/2015 - Health and Wellbeing Board (Item 44)

BETTER CARE FUND UPDATE

To receive a report from the Chief Operating Officer, NHS Trafford Clinical Commissioning Group.

Minutes:

The Associate Director of Commissioning of the Trafford Clinical Commissioning Group provided a verbal update on the progress and the resubmission of the Better Care Fund plans to NHS England.  Notification of whether the scheme is approved will be received on 14th January 2015. 

The Joint Director for CYPS and Community Health Services (Pennine Care Foundation Trust) also advised the Board that work has been progressing with workstreams lined up to ensure work can be picked up quickly.
