Transformation Fund - Re-purpose of the Funding
The Transformation Fund of £22m was awarded by the Greater Manchester Health & Social Care Partnership (GMHSCP) to the Trafford Locality in October 2017. The Fund was to help address issues faced by the local health and social care system. Within this Fund is the new model of primary care initiative which in part was to be achieved via a Primary Care Organisation (PCO). A material condition of the award of this funding was that an external, independent review of the PCO was to be commissioned. The outcome of this was a cessation of the Primary Care Organisation (PCO). However to ensure the CCG continued to support the delivery of a Primary Care Strategy, aimed at delivering primary care at scale as part of the Local Care Alliance model, a stocktake was undertaken on all the schemes in the Transformation Fund to identify those that could be repurposed. This identified £2.3m of funding. The report will request approval for this funding to be repurposed and invested.
